    Summary

Videothoracoscopic parasternal lymphadenectomy is a less traumatic and effective diagnostic method for medial and central breast cancer compared to standard mastectomy. This minimally invasive approach is recommended for specific breast cancer locations.

    Area of Science:

    • Surgical Oncology
    • Thoracic Surgery

    Background:

    • Standard mastectomy by Urban-Holdin is a conventional treatment for breast cancer.
    • Parasternal lymph node involvement is a critical factor in breast cancer staging and treatment.

    Purpose of the Study:

    • To compare the outcomes of videothoracoscopic parasternal lymphadenectomy with standard mastectomy by Urban-Holdin.
    • To evaluate the efficacy and safety of minimally invasive parasternal lymphadenectomy for medial and central breast cancer.

    Main Methods:

    • A comparative analysis of 205 videothoracoscopic parasternal lymphadenectomies (performed between November 1995 and December 1999) and standard mastectomy by Urban-Holdin.
    • The procedure involved mastectomy followed by thoracoports insertion for en-bloc removal of parasternal fat and lymph nodes.
    • Pre-, intra-, and postoperative spirometry and cardiomonitoring were utilized.

    Main Results:

    • Parasternal lymph nodes were involved in 19.5% of patients.
    • Videothoracoscopic parasternal lymphadenectomy was found to be less traumatic and more effective as a diagnostic method than standard mastectomy.
    • The procedure was performed under general anesthesia with artificial lung ventilation and separate lung intubation.

    Conclusions:

    • Videothoracoscopic parasternal lymphadenectomy is a less traumatic and effective diagnostic method for medial and central breast cancer.
    • This minimally invasive technique can be recommended as the method of choice for specific breast cancer locations.
