Background:

  • Hypertriglyceridemia is a known side effect of protease inhibitor (PI) medications, such as ritonavir.
  • Fibrate derivatives, including gemfibrozil, are often prescribed to manage elevated triglyceride levels associated with PI therapy.

Observation:

  • A 35-year-old male patient developed sexual dysfunction approximately three weeks after initiating gemfibrozil treatment.
  • The patient's sexual function improved after discontinuing gemfibrozil.

Findings:

  • The temporal association between gemfibrozil initiation and the onset of sexual dysfunction, along with its subsequent resolution upon drug cessation, suggests a potential causal link.
  • Gemfibrozil may be an underrecognized cause of sexual dysfunction in patients undergoing protease inhibitor therapy.

Implications:

  • Clinicians should consider sexual dysfunction as a potential adverse effect of gemfibrozil in patients treated with protease inhibitors.
  • Further investigation is warranted to elucidate the mechanisms underlying gemfibrozil-induced sexual dysfunction and to inform clinical management strategies.
