Summary

Granulocytic sarcomas can contain needle-like crystals resembling Charcot-Leyden crystals. These inclusions require differentiation from immunoglobulin crystals found in other hematologic malignancies.

Background:

  • Granulocytic sarcoma, a rare extramedullary manifestation of myeloid leukemia, can present with unusual histological features.
  • The presence of crystalline inclusions within tumor cells or necrotic debris requires careful diagnostic evaluation.

Observation:

  • Two cases of granulocytic sarcoma exhibited numerous crystalline inclusions on hematoxylin-eosin staining.
  • These needle-like crystals were located in areas of necrosis and within macrophages.
  • The crystals tested negative for chloroacetate esterase and myeloperoxidase, enzymes typically associated with granulocytic differentiation.

Findings:

  • Electron microscopy revealed dense, bipyramidal structures morphologically identical to Charcot-Leyden crystals.
  • These granulocytic sarcoma-associated crystals are distinct from crystalline immunoglobulin inclusions.

Implications:

  • Recognizing these crystalline inclusions is crucial for accurate diagnosis of granulocytic sarcoma.
  • Distinguishing these from immunoglobulin crystals is essential to avoid misdiagnosis in plasma cell myeloma and lymphoplasmacytic lymphoma.
