Summary

Localized argyria, a rare skin condition, can present with a unique histologic feature termed "pseudo-ochronosis." This finding, characterized by ochre swollen collagen bundles, may be more common than previously thought.

Background:

  • Localized argyria is an uncommon condition presenting as asymptomatic gray or blue macules.
  • Histologically, it typically shows silver granules around eccrine glands, blood vessels, and elastic fibers.
  • Ochre swollen collagen bundles resembling ochronosis have not been previously described.

Observation:

  • This study reports on 5 patients with localized argyria exhibiting the histologic feature of "pseudo-ochronosis."
  • Biopsies were performed on lesions, with some patients undergoing analysis via mass spectrophotometry, scanning electron microscopy, and energy-dispersive x-ray analysis.

Findings:

  • All 5 patients showed typical clinical and histologic signs of localized argyria.
  • Ochre swollen and homogenized collagen bundles were consistently observed.

  • Light microscopy revealed ellipsoid black globules within degenerated collagen in four cases.

    • Implications:

    • The histologic findings suggest that "pseudo-ochronosis" may be an underrecognized feature of localized argyria.
