This case study details a rare instance of a non-functional cervical thyroid gland alongside a functional ectopic lingual thyroid in a patient with hypothyroidism. The unusual coexistence and its underlying causes remain unclear.

Background:

  • Ectopic thyroid tissue is a rare congenital anomaly.
  • Lingual thyroid is the most common site for ectopic thyroid tissue.
  • Non-functional cervical thyroid tissue can occur but is less common.

Observation:

  • A 20-year-old woman presented with hypothyroidism, treated with L-thyroxine.
  • She had a diagnosed basilingual tumor and presented with a spherical basilingual tumor.
  • Oropharyngeal examination revealed a 2.5 cm basilingual tumor.

Findings:

  • Laboratory results indicated elevated TSH (6,280 microIU/ml) despite L-thyroxine treatment.
  • Cervical ultrasonography and CT confirmed a small, hypoechogenic, heterogeneous orthotopic thyroid gland.
  • An I(123) scan showed iodine uptake in the chin area (lingual thyroid) but no significant uptake in the cervical thyroid region.
