Background:

Idiopathic hypoparathyroidism is a rare endocrine disorder characterized by insufficient parathyroid hormone production.
Hypocalcaemia, a common complication, can manifest with various neurological symptoms.

Observation:

A 40-year-old male with idiopathic hypoparathyroidism presented with tetany, proximal weakness, and signs of hypocalcaemia.
Electrophysiological studies revealed a predominantly axonal sensory-motor neuropathy with specific findings on median nerve conduction.

Findings:

Peripheral neuropathy in hypocalcaemia is reversible with treatment.
Normalization of calcium homeostasis led to progressive clinical and electrophysiological improvement in the patient's neuropathy.

This case highlights the critical role of calcium ion concentration in maintaining normal peripheral nerve function.
The findings underscore the importance of early diagnosis and management of hypoparathyroidism to prevent or reverse neurological complications.
