Electronystagmography (ENG) showed limited diagnostic value in elderly dizzy patients. This diagnostic tool failed to differentiate dizziness syndromes or distinguish dizzy individuals from controls in community settings.
Area of Science:
- Neurology
- Otolaryngology
- Geriatrics
Background:
- Dizziness is a common complaint in elderly individuals.
- Accurate diagnosis of dizziness etiology is crucial for effective management.
- Electronystagmography (ENG) is a common test used to evaluate vestibular function.
Purpose of the Study:
- To evaluate the diagnostic utility of electronystagmography (ENG) in a community-based sample of dizzy subjects aged 65 and older.
- To determine if ENG can differentiate between various dizziness syndromes and identify specific causes of dizziness in the elderly.
Main Methods:
- A cohort of 149 dizzy subjects over 65 years old and 96 asymptomatic controls underwent electronystagmography (ENG).
- Clinical diagnoses were established using standardized criteria.
- ENG results were classified as normal or abnormal based on control reference ranges, and diagnostic values (sensitivity, specificity, predictive values) were calculated.
Main Results:
- Central vascular disease was the most frequent diagnosis among dizzy subjects (105/149), while peripheral vestibular disease was infrequent (14/149).
- Spontaneous nystagmus showed a high positive predictive value (95%) for central vascular disease but low sensitivity (18%) and was often clinically detectable.
- ENG demonstrated no significant diagnostic value in differentiating dizzy subjects from controls or distinguishing between various dizziness syndromes.
Conclusions:
- Electronystagmography (ENG) provided no practical diagnostic value in this community-derived sample of elderly individuals experiencing dizziness.
- The findings suggest that ENG is not a reliable tool for diagnosing dizziness in the elderly population within a community setting.
