Summary

Color Doppler ultrasound can distinguish advanced gallbladder cancer (T1b) from sludge, but contrast enhancement improves accuracy. Further Doppler sensitivity is needed for early-stage (T1a) gallbladder cancer detection.

Area of Science:

  • Medical Imaging
  • Oncology
  • Gastroenterology

Background:

  • Gallbladder carcinoma staging is crucial for treatment.
  • Early detection of T1 stage gallbladder cancer remains challenging.
  • Distinguishing tumefactive biliary sludge from early gallbladder cancer requires reliable imaging.

Purpose of the Study:

  • To evaluate the efficacy of color Doppler and contrast-enhanced Doppler ultrasonography in diagnosing T1 stage gallbladder carcinoma.
  • To determine if Doppler signals can differentiate between T1b gallbladder carcinoma and tumefactive biliary sludge.
  • To assess the added value of contrast medium in improving diagnostic confidence.

Main Methods:

  • Retrospective analysis of seven cases with T1 stage gallbladder carcinoma.

Related Experiment Videos

  • Review of color Doppler and contrast-enhanced Doppler ultrasound findings.
  • Comparison of imaging features between gallbladder carcinoma and tumefactive biliary sludge.
  • Main Results:

    • Presence or absence of blood flow signals on color Doppler distinguished T1b gallbladder carcinoma from tumefactive biliary sludge.
    • Contrast medium injection significantly increased diagnostic confidence.
    • Color Doppler sonography had limitations in detecting fine blood flow signals in T1a gallbladder carcinoma.

    Conclusions:

    • Contrast-enhanced Doppler sonography is a valuable tool when color Doppler is ambiguous for T1 gallbladder carcinoma.
    • Improved Doppler sensitivity is necessary for detecting T1a stage gallbladder carcinoma.
    • Doppler ultrasonography plays a role in differentiating early gallbladder malignancies.
