Background:

  • Waldenström's macroglobulinemia (WM) is a rare lymphoproliferative disorder.
  • Cutaneous involvement in WM is uncommon, presenting diagnostic and therapeutic challenges.

Observation:

  • A 76-year-old woman presented with chronic, red, nodular skin lesions on her face.
  • Biopsy revealed a lymphocytic dermal infiltrate with a monoclonal B lymphoid population (IgM-kappa).
  • Diagnosis confirmed as WM with cutaneous localization based on high IgM levels and bone marrow findings.

Findings:

  • Initial radiotherapy was ineffective.
  • Chlorambucil chemotherapy led to rapid resolution of skin lesions.
  • Lesions relapsed but responded to a lower dose of chlorambucil after initial neutropenia.
