    Summary

This study suggests that heparin and high-dose corticosteroids can successfully treat acute anuric lupus nephritis, improving renal function and patient outcomes in most cases. Long-term follow-up showed sustained recovery for most patients.

    Area of Science:

    • Nephrology
    • Rheumatology
    • Immunology

    Background:

    • Acute anuric lupus nephritis is a severe kidney condition characterized by rapid loss of kidney function.
    • Renal biopsy findings in these cases included intracapillary proliferation, epithelial crescents, wire loops, and intracapillary thrombi.
    • Some patients presented with arteriolar involvement, including thrombotic occlusions and fibrinoid necrosis.

    Purpose of the Study:

    • To describe six cases of acute anuric lupus nephritis.
    • To evaluate the efficacy of treatment with heparin, corticosteroids, and dialysis.

    Main Methods:

    • Six patients with acute anuric lupus nephritis were treated with anticoagulant doses of heparin and corticosteroids.
    • Four patients also received dialysis.

    Related Experiment Videos

  • Renal biopsies were performed to assess histopathological changes.
  • Main Results:

    • All patients demonstrated improvement in renal function following treatment.
    • Five out of six patients survived and remained well at long-term follow-up (22-64 months).
    • One patient experienced a second episode of acute renal failure but improved with renewed heparin and high-dose steroid therapy.

    Conclusions:

    • Heparin and high-dose corticosteroids may represent a successful therapeutic strategy for acute anuric lupus nephritis.
    • This treatment approach appears to improve renal function and long-term outcomes.
    • Early intervention with anticoagulation and immunosuppression is crucial for managing severe lupus nephritis.
