Malignant glaucoma following laser iridotomy

Background:
A 69-year-old man presented with malignant glaucoma 10 days after an iridotomy for acute angle-closure glaucoma.
Methods:
The case notes are reviewed, describing his presentation and treatment, including the technique of extracapsular cataract extraction with anterior vitrectomy.
Results:
An excellent visual outcome was achieved with no recurrence of the malignant glaucoma.
Conclusions:
This case illustrates the recently established link between laser iridotomy and malignant glaucoma. Miotics and other possible causes are excluded, a surgical option is described and the importance of assessing central anterior chamber depth in cases of angle closure is emphasised.
