Summary

This study reports a rare case of mixed transcortical aphasia (MTCA) following seizures. The patient experienced temporary language deficits that resolved quickly, indicating a good prognosis for this type of postictal aphasia.

Background:

  • Postictal aphasia, often associated with temporal lobe epilepsy, can manifest in various forms.
  • Mixed transcortical aphasia (MTCA) is a less common type of aphasia characterized by specific language impairments.

Observation:

  • A 67-year-old male, post-left frontal craniotomy for meningioma and treated for generalized tonic-clonic seizures (GTCS), presented in status epilepticus.
  • Upon awakening, the patient exhibited non-fluent speech with palilalia and echolalia, preserved repetition, and impaired other language functions, consistent with MTCA.

Findings:

  • Brain CT revealed no acute abnormalities beyond surgical changes.
  • SPECT scans demonstrated left frontal, cingular, and insular hypoperfusion after language function recovery.
  • The patient's postictal language dysfunction was diagnosed as MTCA, resolving within hours.

Implications:

  • This case highlights a unique presentation of postictal MTCA with a favorable prognosis, even in a patient with frontal lobe involvement.
  • Understanding such transient aphasic episodes is crucial for accurate diagnosis and management of post-seizure neurological deficits.
  • Further research into the neurovascular mechanisms underlying transient postictal aphasia is warranted.
