• Fluorescein angiography showed hyperfluorescence in specific retinal lesions.
• Unilateral internal ophthalmoplegia, a very rare finding, was noted.

Implications:

• This case underscores the diverse ocular manifestations of sarcoidosis.
• Prompt diagnosis and prolonged corticosteroid therapy are crucial for managing sarcoidosis retinopathy.
