Summary

A rare case of osteogenic sarcoma of the urinary bladder is presented. Initially diagnosed as transitional cell carcinoma, the tumor recurred as a distinct bone-forming sarcoma.

Background:

  • Transitional cell carcinoma (TCC) is the most common bladder cancer.
  • Metastatic or secondary bone-forming tumors of the bladder are exceptionally rare.

Observation:

  • A 25-year-old man presented with hematuria and a bladder tumor.
  • Initial diagnosis was TCC, treated with resection and radiotherapy.
  • Recurrent tumor after radiotherapy involved pelvic organs and sigmoid colon.

Findings:

  • Histopathologic examination of the recurrent tumor revealed predominantly osteogenic sarcoma.
  • This represents an extremely rare presentation of primary osteogenic sarcoma in the urinary bladder.

Implications:

  • Highlights the importance of considering rare diagnoses in recurrent or unusual tumor presentations.
  • Suggests potential for divergent differentiation in bladder tumors or rare primary bone sarcomas arising in the bladder.
