Area of Science:
- Otolaryngology
- Oncology
- Radiology
Background:
- Primary isolated sphenoid sinus carcinoma is an extremely rare diagnosis.
- Sphenoid sinus malignancies often present with vague symptoms, delaying diagnosis.
Observation:
- A 68-year-old man presented with persistent headache and diplopia due to complete ophthalmoplegia.
- Paranasal sinus tomography revealed a soft-tissue mass in the sphenoid sinus, invading the cavernous sinuses.
Findings:
- Endoscopic intranasal biopsy confirmed adenocarcinoma.
- The patient responded to radiotherapy, with symptom resolution.
- However, multiple bone metastases developed six months post-diagnosis.
Implications:
- This case underscores the rarity of primary sphenoid sinus adenocarcinoma.
- Early recognition of sphenoid sinus lesions is crucial for timely management.
- The aggressive metastatic potential highlights the need for vigilant follow-up.
