Cardiac catheterization is safe for morbidly obese patients with suspected coronary heart disease (CHD). Noninvasive tests are often inaccurate for this population, making cardiac catheterization a more reliable diagnostic tool.
Area of Science:
- Cardiology
- Medical Imaging
- Obesity Medicine
Background:
- The diagnostic accuracy of cardiac catheterization and coronary angiography in morbidly obese patients with suspected coronary heart disease (CHD) requires further investigation.
- Morbid obesity is defined as a body mass ≥ 145 kg and body mass index ≥ 40 kg/m².
Purpose of the Study:
- To evaluate the safety and effectiveness of cardiac catheterization in morbidly obese patients.
- To compare the diagnostic accuracy of noninvasive tests versus cardiac catheterization for CHD in this population.
Main Methods:
- Retrospective analysis of 4,978 patients undergoing diagnostic cardiac catheterization.
- Identification of 110 morbidly obese patients for comparative analysis.
- Assessment of noninvasive test sensitivity and specificity for CHD in morbidly obese patients.
Main Results:
- Morbidly obese patients had a lower prevalence of CHD (45%) compared to other patients (72%), indicating a higher rate of false positive noninvasive tests.
- Noninvasive tests demonstrated low sensitivity (75%) and specificity (39%) for CHD in this cohort.
- Cardiac catheterization in morbidly obese patients utilized radial access (66%) and femoral closure devices (24%) more frequently.
- Complication rates (major and minor) were similar between morbidly obese and non-obese patient groups.
Conclusions:
- Cardiac catheterization, particularly with radial artery access or femoral closure devices, is a safe and effective method for evaluating CHD in morbidly obese individuals.
- Noninvasive testing for CHD in morbidly obese patients is often inconclusive and can be misleading.
- Diagnostic cardiac catheterization offers a more definitive assessment of coronary heart disease in the morbidly obese population.
