    Area of Science:

    • Internal Medicine
    • Gastroenterology
    • Infectious Diseases

    Background:

    • Abdominal pain and fever can indicate serious underlying conditions.
    • Hepatosplenomegaly and hemoperitoneum are critical findings requiring urgent evaluation.
    • Infectious Mononucleosis (IM) is typically a benign viral illness but can present with atypical complications.

    Purpose of the Study:

    • To report a rare case of severe abdominal complications secondary to Infectious Mononucleosis.
    • To highlight the diagnostic challenges and management of IM presenting with acute abdominal symptoms.
    • To emphasize the importance of considering IM in the differential diagnosis of febrile syndromes with abdominal pain.

    Main Methods:

    • Case report of a patient presenting with febrile syndrome and abdominal pain.

    Related Experiment Videos

  • Diagnostic imaging including abdominal ultrasound and Computerized Tomography (CT).
  • Surgical intervention (splenectomy) and histopathological examination of the spleen.
  • Main Results:

    • The patient presented with a 4-day history of fever and abdominal pain, with imaging revealing hepatosplenomegaly and hemoperitoneum.
    • Emergency splenectomy was performed, followed by the development of acute pancreatitis and a low-output fistula.
    • Histopathology confirmed Infectious Mononucleosis (IM), supported by serologic tests.

    Conclusions:

    • Infectious Mononucleosis can manifest with severe, life-threatening abdominal complications.
    • Early diagnosis and appropriate management, including surgical intervention and antibiotics, are crucial for favorable outcomes.
    • This case underscores the diverse clinical spectrum of IM and the need for a high index of suspicion in patients with unexplained abdominal symptoms.
