Summary
Transhiatal esophagectomy (THE) is a safe and effective surgical procedure for esophageal diseases. This study shows reduced mortality and comparable long-term outcomes for patients undergoing THE.
Area of Science:
- Gastroenterology
- Surgical Oncology
- Thoracic Surgery
Background:
- Presents 18-year experience with transhiatal esophagectomy (THE).
- Covers both benign and malignant esophageal diseases.
Purpose of the Study:
- Evaluate the safety and efficacy of transhiatal esophagectomy.
- Analyze long-term outcomes and survival rates.
Main Methods:
- Retrospective analysis of 411 consecutive patients undergoing THE (1982-2000).
- Benign (n=44) and malignant (n=367) disease groups.
- Follow-up for surviving patients averaged 30.4 months.
Main Results:
- Overall operative mortality: 11%, reduced to 6% in the last 111 patients.
- Mortality in benign group <5%. Respiratory complications were most frequent.
- 14% nonfatal anastomotic leaks. 2, 5, 10-year survival for carcinoma: 54%, 38%, 18%.
Conclusions:
- Transhiatal esophagectomy is a safe and effective surgical option.
- Achieved results are comparable to published series.
- Demonstrates favorable long-term outcomes for esophageal cancer patients.
