Summary

A patient with bipolar disorder developed serotonin syndrome (SS) after adding olanzapine to lithium and citalopram. This case highlights the risk of SS when combining atypical antipsychotics with serotonergic drugs.

Area of Science:

  • Neuroscience
  • Psychiatry
  • Clinical Pharmacology

Background:

  • Serotonin syndrome (SS) is a potentially life-threatening condition caused by excessive serotonergic agonism.
  • Bipolar affective disorder is a mood disorder requiring complex pharmacotherapy.
  • Managing mood disorders often involves multiple psychotropic medications, increasing the risk of drug interactions.

Observation:

  • A patient diagnosed with bipolar affective disorder was treated with lithium and citalopram.
  • Olanzapine, an atypical antipsychotic, was added to the patient's medication regimen.
  • The patient subsequently developed symptoms consistent with serotonin syndrome.

Findings:

  • This case presents the first reported instance implicating olanzapine in the development of serotonin syndrome.

Related Experiment Videos

  • The combination of lithium, citalopram (a serotonergic agent), and olanzapine (an atypical antipsychotic) appears to have precipitated SS.
  • The patient's clinical presentation resolved upon discontinuation of the offending agents.
  • Implications:

    • Clinicians must exercise caution when prescribing atypical antipsychotics, such as olanzapine, concurrently with serotonergic medications.
    • Awareness of this potential drug interaction is crucial for preventing SS in patients with psychiatric disorders.
    • Further research may be warranted to elucidate the specific mechanisms underlying olanzapine-induced SS.
