Background:

The standard approach for mitral valve exposure involves a longitudinal right lateral left atrial incision.
Alternative techniques exist, but the left atrial roof incision, though shortest, faced challenges with technical difficulty and inadequacy.

To present a modified left atrial roof incision technique for mitral valve surgery.
To evaluate the safety, ease of use, and adaptability of this modified approach in a clinical setting.

A modified left atrial roof incision technique was developed and applied.
