Summary
A rare malignant sacrococcygeal yolk sac tumor was surgically removed from a young child. Postoperative chemotherapy was initiated, highlighting a successful treatment approach for this rare germ-cell tumor.

Observation:
- A 2.5-year-old female presented with a presacral mass and overlying skin ulceration.
- Imaging revealed a large presacral mass extending into the pelvis, with elevated serum AFP levels.
- No retroperitoneal lymph node or liver involvement was detected.
Findings:
- Complete tumor excision (in-toto) with the coccyx was achieved via a posterior approach.
- Postoperative VAC (Vincristine, Actinomycin-D, Cyclophosphamide) chemotherapy was administered 2 weeks after surgery.
Implications:
- This case demonstrates a feasible surgical and chemotherapeutic strategy for malignant sacrococcygeal yolk sac tumors.
- Further research into rare extra-gonadal germ-cell tumors is warranted to improve patient outcomes.
