Background:

  • Giant cell angiofibroma (GCA) is a rare benign mesenchymal neoplasm.
  • Typically found in the orbit, GCA can present in extraorbital locations.

Observation:

  • A 47-year-old male presented with a 6 cm subcutaneous forearm tumor.
  • Histological examination revealed a GCA with characteristic vascular patterns and atypical cells.
  • Immunohistochemistry showed diffuse vimentin and CD34 expression.

Findings:

  • The tumor exhibited a unique histological pattern with small, hyalinized vessels and a mix of mononuclear and bizarre lesional cells.
  • Immunohistochemical analysis confirmed GCA, with negative staining for cytokeratins, bcl-2, desmin, actin, VWF, S100, alpha-1-antitrypsin, CD99, lysozyme, and FXIIIa.
  • The patient experienced no recurrence after 13 months of follow-up.

Implications:

  • This case expands the known anatomical distribution of giant cell angiofibroma.
  • It underscores the importance of considering GCA in the differential diagnosis of soft tissue tumors.
  • The findings support GCA as a benign entity with favorable prognosis, even in extraorbital sites.
