    Summary

    Diagnosing ileal conduit obstruction early is possible by analyzing pressure data. Increased residual urine, even slightly above normal, can indicate early blockage before dilation occurs.

    Background:

    • Ileal conduits are common urinary diversions.
    • Early diagnosis of obstruction is crucial to prevent complications.
    • Current diagnostic methods may not detect subtle changes.

    Observation:

    • A continuously perfused catheter technique was employed.
    • Pressures within the ileal conduit during filling and emptying were measured.
    • Stomal resistance to flow was assessed.

    Findings:

    • Obstructed conduits showed significantly higher mean values for residual urine, resting pressure, voiding pressure, and stomal profile pressure compared to non-obstructed conduits.
    • A small increase in residual urine above 4.0 ± 0.9 cc may signal early obstruction.
    • Stomal caliber did not consistently correlate with the degree of obstruction.

    Implications:

    • This technique aids in the early detection of ileal conduit obstruction.
    • Monitoring pressure dynamics can prevent upper urinary tract damage.
    • Further analysis of comprehensive pressure data is recommended for accurate diagnosis.
