Spinal extradural angiolipomas are rare tumors. This case highlights acute myelopathy presentation and successful surgical resection in an elderly patient.

Area of Science:

  • Neurosurgery
  • Oncology
  • Spinal Cord Diseases

Background:

  • Spinal extradural angiolipomas are rare benign tumors, comprising 0.14-1.2% of all spinal neoplasms.
  • Typically located in the thoracic extradural space, they often present as slowly progressive myelopathy.

Observation:

  • An 85-year-old male presented with acute bilateral lower limb weakness and sphincter disturbance.
  • MRI revealed a posterior extradural lesion at L1-L2, compressing the conus medullaris and cauda equina.

Findings:

  • Urgent surgical treatment involved L1-L2 laminectomy for total tumor resection.
  • Pathological examination confirmed the lesion as an angiolipoma.

Implications:

  • This case underscores the possibility of acute presentation of spinal extradural angiolipomas.
