Summary

Attachment site failure in aortic aneurysms transmits systemic pressure to the sac, causing endotension even without endoleak. Cuff exclusion effectively reduces aneurysm sac pressure (ASP), aiding intervention decisions.

Area of Science:

  • Vascular Surgery
  • Biomedical Engineering
  • Medical Device Design

Background:

  • Endovascular aneurysm repair (EVAR) aims to exclude aortic aneurysms.
  • Attachment site failure is a complication of EVAR.
  • Endotension, elevated aneurysm sac pressure (ASP), can occur post-EVAR.

Purpose of the Study:

  • To develop an experimental model for endotension.
  • To determine if attachment site failure without endoleak increases ASP.
  • To evaluate the impact of cuff exclusion on ASP.

Main Methods:

  • Created infrarenal aortic aneurysms in canines.
  • Utilized stent graft exclusion and induced attachment site failure.
  • Measured ASP and systemic blood pressure (BP), calculating the ASP/BP ratio.

Related Experiment Videos

  • Administered norepinephrine to assess pressure responses.
  • Main Results:

    • No endoleak detected by imaging in either group.
    • Group II (attachment site failure) showed significantly higher ASP/BP ratios (1.01) than Group I (0.39).
    • Cuff exclusion in Group II reduced ASP/BP from 0.98 to 0.46.

    Conclusions:

    • Attachment site failure transmits systemic pressure to the aneurysm sac, causing endotension.
    • Cuff exclusion of attachment site failure effectively lowers ASP.
    • ASP monitoring may guide post-EVAR intervention decisions.
