Background:

  • Organ transplantation, particularly heart transplantation, is associated with an increased risk of secondary malignancies.
  • Precancerous lesions and malignant tumors, especially skin cancers, are common complications in transplant recipients.

Purpose of the Study:

  • To evaluate the incidence and characteristics of neoplasias in patients who underwent heart transplantation.
  • To determine the risk factors associated with tumor development post-heart transplantation.

Main Methods:

  • A cohort of 241 heart transplant patients (1990-2000) was retrospectively evaluated for neoplasia development.
  • A standardized dermatological examination was performed on 156 surviving patients.

  • Malignancy was detected in 28 patients (17.9%) after an average of 4.98 years post-transplant.
  • Skin was the most frequently affected site (64%), followed by the urinary/genital tract (18%), respiratory (7%), gastrointestinal (7%), and breast (4%).
  • Patients who developed tumors were significantly older (average 59.5 years) than the overall cohort mean age (49.8 years).

Conclusions:

  • Heart transplantation at least doubles the risk of developing cancer.
  • No correlation was found between malignancy and HLA matching, immunosuppressive drug details, or rejection episodes.
  • Regular dermatological examinations are recommended for heart transplant recipients due to the high incidence of skin tumors.
