Kidney Transplant I: Introduction01:28

Kidney Transplant I: Introduction

599
A kidney transplant is a surgical approach that involves replacing a non-functioning kidney with a healthy one from a donor. This procedure is often a treatment option for end-stage renal disease (ESRD) patients. The method requires careful recipient selection, including evaluating various medical and psychosocial factors. These criteria vary between transplant centers but generally include assessments of the patient's overall health, adherence to medical recommendations, and lifestyle...

Kidney Transplant II: Surgical Procedure

572
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Summary

Live donor kidney transplant offers superior survival rates compared to deceased donor transplants. Hand-assisted laparoscopic donor nephrectomy ensures a fast recovery for the donor and immediate function for the recipient kidney.

Area of Science:

  • Nephrology
  • Surgical Innovation
  • Transplantation Medicine

Background:

  • Live donor renal transplantation is preferred over cadaveric transplantation due to better patient and graft survival.
  • Minimally invasive techniques like laparoscopic donor nephrectomy reduce surgical morbidity and recovery time.

Observation:

  • A 37-year-old hepatitis-B surface antigen carrier donated a kidney to his brother.
  • The donor and recipient showed good compatibility via cross-matching and human leukocyte antigen testing.
  • Hand-assisted laparoscopic left donor nephrectomy was successfully performed.

Findings:

  • The surgery resulted in minimal warm ischemia time (4.5 minutes).
  • The transplanted kidney functioned immediately.
  • The donor recovered well and was discharged on postoperative Day 3.

Implications:

  • Hand-assisted laparoscopic donor nephrectomy is a safe and effective procedure for live kidney donation.
  • This technique facilitates rapid recovery and optimal graft function.
  • It supports the advantages of live donor kidney transplantation, even in hepatitis-B carriers.
