Aneurysmal bone cysts (ABCs) rarely affect wrist bones. This case details an ABC in the hamate bone, treated successfully with surgery and bone grafting, leading to wrist stability.

Background:

  • Aneurysmal bone cysts (ABCs) are benign, expansile, osteolytic bone lesions.
  • ABCs typically occur in the metaphysis of long bones and vertebrae.
  • Carpal bone involvement by ABCs is exceptionally rare.

Observation:

  • An 18-year-old male presented with an aneurysmal bone cyst in the left hamate bone.
  • The cyst caused extensive cortical destruction of the hamate.

Findings:

  • Surgical intervention included curettage and bone grafting.
  • Due to severe cortical destruction, complete excision of the hamate was necessary.
  • The patient achieved wrist stability at a 3-year follow-up.

Implications:

  • This case highlights the possibility of aneurysmal bone cysts occurring in carpal bones.
  • Aggressive surgical management may be required for extensive lesions.
  • Successful treatment can lead to long-term wrist stability despite significant bone loss.
