Summary

A complex heart defect involving congenitally corrected transposition of the great arteries and dextrocardia was successfully treated with a Senning plus Rastelli operation. The patient showed good biventricular function post-surgery, indicating a positive outcome for this rare condition.

Area of Science:

  • Cardiovascular Surgery
  • Pediatric Cardiology
  • Congenital Heart Disease

Background:

  • Congenitally corrected transposition of the great arteries (S,L,L) with dextrocardia is a rare complex congenital heart defect.
  • Major aortopulmonary collaterals and arborization defects in diminutive central pulmonary arteries present significant surgical challenges.

Observation:

  • A patient with this rare combination underwent a Senning plus Rastelli operation.
  • The patient required four preparatory procedures, including bilateral unifocalizations of aortopulmonary artery collaterals.

Findings:

  • The double switch operation resulted in a pulmonary artery to systemic pressure ratio of 0.6.
  • Post-operative echocardiogram at 3 months revealed good biventricular function.

Related Experiment Videos

Implications:

  • This case demonstrates the feasibility of complex surgical correction for rare congenital heart anomalies.
  • Successful biventricular repair in such complex cases offers hope for improved long-term outcomes.
  • Further research into managing major aortopulmonary collaterals and pulmonary artery hypoplasia is warranted.
