Background:

  • Subungual melanomas are rare, accounting for 2-3% of cutaneous melanomas in White populations.
  • Traditional treatment involved amputation proximal to the distal interphalangeal joint.
  • Limited excision with 3D histology is a newer approach for acral melanomas.

Purpose of the Study:

  • To evaluate prognostic relevance of clinical parameters in subungual melanoma.
  • To compare outcomes of different surgical management strategies for subungual melanoma.

Main Methods:

  • Retrospective analysis of 62 subungual melanoma patients (1980-1999).
  • Comparison of two groups: amputation (n=31) vs. "functional" surgery with local excision (n=31).
  • Median follow-up of 55 months for amputation group and 54 months for functional surgery group.

  • Univariate analysis: invasion level, ulceration, and tumor thickness were significant for recurrence-free survival.
