Purpose of the Study:

  • To compare histologic findings and outcomes of focal versus bilateral parathyroid exploration in patients with primary sporadic hyperparathyroidism and a solitary adenoma.
  • To evaluate the efficacy of intraoperative parathyroid hormone testing in both surgical approaches.

Main Methods:

  • Eighty patients with primary hyperparathyroidism and a preoperatively identified solitary abnormal parathyroid gland were included.
  • Patients underwent either standard bilateral neck exploration (45 patients) or focal neck exploration (35 patients).

  • All patients had intraoperative parathyroid hormone testing and were followed for a mean of 17 months.

    • Focal exploration identified a single adenoma in all patients, whereas bilateral exploration found single adenomas in 84% and multiple parathyroid tumors in 15% of patients.
    • Sestamibi scanning and intraoperative parathyroid hormone assay accuracy were higher in the focal group (96.9% and 94.3%) compared to the bilateral group (87% and 84%).
    • All patients achieved normocalcemia postoperatively.

    Conclusions:

    • Bilateral parathyroid exploration revealed a higher incidence of multiple parathyroid tumors compared to the focal approach.
    • Despite differences in histologic findings, all patients achieved immediate postoperative normocalcemia.
    • Long-term surveillance is required to ascertain the recurrence rates of primary hyperparathyroidism in patients treated with focal neck exploration.
