Physician group builds tools to help practitioners boost diabetes outcomes

A group practice has developed two new tools to help physicians and patients take DM matters into their own hands. The Diabetes Patient Report Card is designed to help patients stay on track with periodic blood testing and other routine preventive tasks, and the Diabetes Physician Audit Tool provides a simple way for physicians to measure their own performance on a range of measures.
