    Summary

    Closure of patent ductus arteriosus with a hypoplastic right lung can be achieved via median sternotomy. An anterior approach to the left ductus is suitable when left thoracotomy is not an option.

    Observation:

    • Median sternotomy was employed for PDA closure in a patient with a hypoplastic right lung.
    • The anterior approach to the left ductus arteriosus was considered.

    Findings:

    • Median sternotomy provides adequate access for PDA closure even with right lung hypoplasia.
    • The anterior approach to the left ductus is a viable alternative when left thoracotomy is contraindicated.

    Implications:

    • This approach may offer a safe and effective alternative for PDA closure in complex pediatric cases.
    • Further studies are warranted to evaluate the long-term outcomes of this surgical strategy.
