Area of Science:

  • Infectious Diseases
  • Surgical Pathology
  • Critical Care Medicine

Background:

  • Psoas abscess (PA) is a serious infection requiring prompt diagnosis and management.
  • Septic shock in PA patients presents unique diagnostic and prognostic challenges.

Purpose of the Study:

  • To elucidate the clinical features, diagnostic approaches, and treatment strategies for psoas abscess.
  • To specifically analyze the impact of septic shock on the presentation and outcomes of PA.

Main Methods:

  • Retrospective analysis of 17 patients diagnosed with psoas abscess.
  • Treatment involved intravenous antibiotics and abscess drainage (surgical or percutaneous ultrasound-guided).

Main Results:

  • Fever, back/flank/abdominal pain, and hip flexion contracture were common presenting symptoms.

Related Experiment Videos

  • Patients with septic shock exhibited less typical symptoms (44%) compared to those without (100%).
  • Symptom duration was significantly shorter in septic shock patients (median 2 days vs. 18.5 days), with higher mortality (33% vs. 0%).
  • Conclusions:

    • Psoas abscess patients with septic shock tend to have nonspecific symptoms and a more occult clinical course.
    • Delayed diagnosis and treatment in PA, especially with septic shock, correlate with worse clinical outcomes.
    • Increased clinical awareness is vital for earlier diagnosis and improved patient survival and recovery.
