Laparoscopic partial nephrectomy with adrenalectomy is effective for upper pole renal tumors involving the adrenal gland. This minimally invasive approach preserves oncologic principles and achieves good outcomes.

Area of Science:

  • Urology
  • Surgical Oncology
  • Minimally Invasive Surgery

Background:

  • Upper pole renal tumors can involve the adrenal gland, necessitating simultaneous removal.
  • Laparoscopic partial nephrectomy (LPN) is standard for kidney tumors, but concomitant adrenalectomy during LPN is less described.

Observation:

  • Four patients with upper pole renal tumors and suspected adrenal involvement underwent LPN with ipsilateral adrenalectomy.
  • Preoperative imaging guided the surgical approach, with tumors abutting or involving the adrenal gland.
  • A transperitoneal laparoscopic technique was employed, involving en bloc excision of the adrenal gland followed by LPN.

Findings:

  • All procedures were completed laparoscopically without complications.
  • Mean operative time was 3.9 hours, with minimal blood loss and a short hospital stay.

Related Experiment Videos

  • Pathology confirmed negative margins for all renal tumors and adrenal glands, with no malignancy in three adrenal glands.
  • Implications:

    • Laparoscopic partial nephrectomy with concomitant adrenalectomy is a feasible and oncologically sound option for selected patients.
    • This technique offers a minimally invasive solution for complex upper pole renal tumors with adrenal involvement.
    • Further studies can validate this approach in larger patient cohorts.
