Background:

  • Cutaneous angiosarcoma is a rare vascular tumor with a poor prognosis due to high metastatic potential.
  • This condition often presents diagnostic challenges, mimicking other benign skin conditions.

Observation:

  • A 57-year-old man presented with an 8-month history of a progressively enlarging, asymptomatic red patch on his left periorbital region.
  • The lesion was initially misdiagnosed as angiolupoid leishmaniasis, insect bites, or cellulitis, and inadequately treated with topical antibiotics and steroids.

Findings:

  • Skin biopsy revealed a poorly differentiated angiosarcoma.
  • Histological and immunohistochemical examinations confirmed the diagnosis.
