    Summary

Pulmonary embolism can rarely lead to paradoxical embolism, especially when a patent foramen ovale is present. This case highlights the diagnostic challenges of this rare cardiovascular complication.

    Area of Science:

    • Cardiology
    • Vascular Medicine
    • Diagnostic Imaging

    Background:

    • Pulmonary embolism (PE) is a significant cause of morbidity and mortality.
    • Paradoxical embolism, where an embolus travels from the venous to the arterial system, is a rare complication.
    • Patent foramen ovale (PFO) is an atrial septal defect that can facilitate paradoxical embolism.

    Observation:

    • A case of pulmonary embolism complicated by paradoxical embolism is presented.
    • The patient's presentation and diagnostic workup are detailed.
    • The rarity of diagnosing paradoxical embolism during a patient's lifetime is emphasized.

    Findings:

    • Patent foramen ovale is a common condition.
    • The coexistence of pulmonary embolism and a patent foramen ovale increases the likelihood of paradoxical embolism.
    • This case underscores the importance of considering paradoxical embolism in specific clinical scenarios.

    Implications:

    • Increased awareness of paradoxical embolism can improve diagnostic rates.
    • Understanding the interplay between PFO and PE is crucial for risk stratification.
    • Further research into the diagnostic modalities and management of paradoxical embolism is warranted.
