Background:

  • Leukocytosis, characterized by elevated white blood cell counts, requires careful diagnosis to differentiate various hematological disorders.
  • Chronic myeloid leukemia (CML) and chronic neutrophilic leukemia (CNL) are distinct conditions presenting with increased neutrophils.

Observation:

  • A 65-year-old male presented with marked leukocytosis (37,500 leukocytes/microliter), predominantly mature neutrophil-like cells.
  • Morphological examination revealed toxic granules and Döhle bodies in neutrophils.
  • Neutrophil function tests showed elevated superoxide production, nitroblue tetrazolium reduction, and phagocytosis.

Findings:

  • High neutrophil alkaline phosphatase score and normal bone marrow karyotype supported a diagnosis of chronic neutrophilic leukemia over chronic myeloid leukemia.
