Background:

Blackfan-Diamond syndrome is a rare inherited bone marrow failure disorder.
Patients often require lifelong blood transfusions and iron chelation therapy.
The syndrome is associated with an increased risk of certain cancers.

Observation:

A 24-year-old patient with Blackfan-Diamond syndrome developed Hodgkin's disease.
The patient experienced transfusion dependence, hemochromatosis despite chelation, and infectious complications.
This occurred after initial corticosensitivity and failure of androgen therapy.

Findings:

Literature review indicates a higher incidence of malignancies in Blackfan-Diamond syndrome compared to other hemosiderosis conditions.
Three cases of leukemia were noted in Blackfan-Diamond syndrome patients.
