Area of Science:

  • Gastroenterology and Infectious Diseases
  • Hematology
  • Rheumatology

Background:

  • Whipple's disease is a rare bacterial infection.
  • It often presents with gastrointestinal and systemic symptoms.

Observation:

  • A 57-year-old woman presented with 5 years of migratory arthralgia, weight loss, and anemia.
  • Initial diagnosis was challenging due to atypical presentation.

Findings:

  • Abdominal lymph node biopsy confirmed Whipple's disease.
  • Retrospective bone marrow biopsy revealed characteristic PAS-positive sickle-formed particles in macrophages.

Implications:

  • This suggests bone marrow biopsy with PAS staining is valuable for diagnosing atypical Whipple's disease.
  • Early diagnosis can improve patient outcomes for this rare condition.
