Summary

Unilateral hyperhidrosis after ischemic stroke is rare. A hypothalamic lesion, potentially linked to the posterior cerebral artery, may cause this persistent sweating.

Background:

  • Ischemic stroke can lead to various neurological deficits.
  • Unilateral hyperhidrosis is an uncommon post-stroke symptom.

Observation:

  • A 64-year-old woman presented with right hemiplegia, motor aphasia, and unilateral hyperhidrosis.
  • Excessive sweating was predominantly on the right side of her body, particularly the face.
  • This unilateral sweating persisted for over two months.

Findings:

  • MRI revealed hemorrhagic infarctions in the left basal ganglia, internal capsule, thalamus, hypothalamus, and cerebral peduncle.
  • 123I-IMP SPECT showed hypoperfusion in the left striatum, thalamus, occipital cortex, and right cerebellum.
  • Cerebral angiography indicated arteriosclerotic changes but no occlusion in the left posterior cerebral artery.
