A new complication, bleeding from neovascularization around the Descemet window, can occur months after deep sclerectomy (DS) with SK-gel implantation. Careful gonioscopy is recommended to prevent hyphema during examination in glaucoma patients.

Area of Science:

  • Ophthalmology
  • Glaucoma Surgery

Background:

  • Deep sclerectomy (DS) with SK-gel implantation is a surgical treatment for glaucoma.
  • Combined phacoemulsification and DS with SK-gel is performed for patients with cataract and glaucoma.

Observation:

  • Two patients developed hyphema during gonioscopic examination 7 and 8 months post-surgery.
  • Bleeding originated from newly formed vessels around the Descemet window.
  • Hyphema was controlled by direct pressure with the goniolens.

Findings:

  • Neovascularization around the Descemet window can occur after DS with SK-gel.
  • Vessels at the Descemet window are susceptible to bleeding during gonioscopy, even months after surgery.

Implications:

Related Experiment Videos

  • This complication highlights a potential risk associated with gonioscopy after DS.
  • Careful gonioscopic examination is crucial to avoid inducing hyphema in patients who have undergone DS.
  • Awareness of this complication is important for managing patients post-glaucoma surgery.
