Summary

The sucrose permeability test may help detect gastrointestinal issues in Behçet's disease (BD). Patients with active BD showed higher gut permeability than those with inactive disease or healthy controls.

Background:

  • Behçet's disease (BD) is a chronic systemic vasculitis with unknown causes.
  • Gastrointestinal (GI) involvement is a common complication in BD patients.
  • Current methods for detecting GI mucosal damage in BD have limitations.

Purpose of the Study:

  • To evaluate the predictive value of the sucrose permeability test for identifying GI mucosal damage in BD patients.
  • To compare GI permeability between active BD, inactive BD, and healthy controls.
  • To correlate GI permeability findings with endoscopic results.

Main Methods:

  • A cohort of 26 male BD patients and 21 age/sex-matched controls underwent upper GI permeability testing using sucrose.
  • Patients were categorized into active (elevated ESR/CRP or active criteria) and inactive disease groups.

  • 25 patients also underwent endoscopic examination for GI assessment.
  • Main Results:

    • BD patients exhibited significantly higher urine sucrose levels compared to controls (P=0.0001).
    • Patients with active BD showed markedly increased permeability versus inactive BD (P<0.0001).
    • No significant difference in permeability was observed between inactive BD patients and controls.
    • Endoscopic findings were non-specific and similar in active and inactive BD.

    Conclusions:

    • Increased upper GI permeability is present in BD patients, particularly those with active disease.
    • The sucrose permeability test may serve as a non-invasive marker for GI mucosal involvement in BD.
    • The observed permeability increase is not linked to specific endoscopic lesions in BD.
    • Further large-scale studies are needed to confirm the utility of the sucrose permeability test in BD.
