Sentinel node biopsy is feasible in most bladder cancer patients undergoing cystectomy. This technique can identify lymph node metastasis outside the obturator fossa, aiding staging and treatment decisions.

Area of Science:

  • Urology
  • Oncology
  • Surgical Pathology

Background:

  • Lymph node status is a critical prognostic indicator in muscle-invasive bladder cancer.
  • The role and extent of lymphadenectomy during cystectomy remain subjects of debate.

Purpose of the Study:

  • To assess the feasibility of the sentinel node (SN) concept in bladder cancer.
  • To correlate SN findings with the status of excised regional lymph nodes.

Main Methods:

  • Preoperative lymphoscintigraphy, peroperative dye detection (Patent Blue), and dynamic lymphoscintigraphy were performed in 26 patients.
  • Radiotracer substances were injected adjacent to the detrusor muscle tumour.

Main Results:

  • Sentinel nodes were successfully detected in 21 out of 26 patients.

Related Experiment Videos

  • Lymph node metastasis was found in 8 patients, with 5 cases showing metastasis outside the obturator fossa.
  • One false negative SN result occurred in a patient with multifocal tumour.
  • Conclusions:

    • Sentinel node detection is achievable in the majority of bladder cancer patients undergoing cystectomy.
    • The utility of SN detection for identifying metastasis outside the obturator fossa requires further research.
