Background:

Idiopathic retroperitoneal hematomas are rare and can present with varied clinical manifestations.
Abdominal masses can arise from diverse etiologies, necessitating thorough diagnostic evaluation.

Observation:

A case of an asymptomatic patient presenting with a retroperitoneal hematoma is described.
The hematoma mimicked an abdominal tumor on initial presentation, posing a diagnostic challenge.

Findings:

The condition was diagnosed as an idiopathic retroperitoneal hematoma.
A comprehensive literature review found no prior reports of similar asymptomatic presentations.

Implications:

This case highlights the importance of considering rare differential diagnoses for abdominal masses.
Further research may be warranted to understand the pathogenesis and optimal management of such idiopathic cases.
