Summary

Financial incentives may influence mild head trauma (MHT) patients' reported memory deficits. Poor motivation, not brain injury, may explain exaggerated Rey AVLT recognition memory impairments in some MHT individuals.

Area of Science:

  • Neuropsychology
  • Cognitive Psychology
  • Clinical Psychology

Background:

  • Financial incentives can impact patient reporting in clinical assessments.
  • Mild head trauma (MHT) patients sometimes exhibit exaggerated cognitive deficits.
  • Rey Auditory Verbal Learning Test (AVLT) is a common measure for memory assessment.

Purpose of the Study:

  • To investigate the effect of financial incentives on Rey AVLT recognition memory performance in MHT patients.
  • To differentiate between genuine memory impairment and motivation-related exaggeration of deficits.
  • To compare MHT patients with and without neurological brain dysfunction.

Main Methods:

  • Assessed Rey AVLT recognition memory performance in MHT patients receiving financial incentives.

Related Experiment Videos

  • Compared MHT patients to those with neurologically documented brain dysfunction without incentives.
  • Subdivided MHT group based on motivation assessed by the Portland Digit Recognition Test (PDRT).
  • Equated groups on Rey AVLT acquisition, age, and education.
  • Main Results:

    • MHT subgroup with poor PDRT scores showed significantly impaired recognition memory.
    • Prevalence of poor recognition scores (<6 correct) was 27% in MHT group vs. 5% in brain dysfunction group.
    • Recognition memory impairment in MHT patients may be linked to motivation to exaggerate deficits.

    Conclusions:

    • Severe Rey AVLT recognition memory impairment in MHT patients may reflect motivation to exaggerate deficits.
    • Financial incentives might play a role in the reporting of memory deficits.
    • Lack of neurological evidence of brain injury suggests motivation as a key factor in some MHT memory impairments.
