Background:

  • Tricyclic antidepressants (TCAs) can cause withdrawal symptoms upon discontinuation.

Observation:

  • Two patients experienced withdrawal symptoms after discontinuing clomipramine.
  • A male patient on 75 mg/d developed flu-like symptoms and insomnia after abrupt cessation.
  • A female patient on 25 mg/d experienced severe insomnia, anxiety, and agitation after discontinuation.

Findings:

  • Withdrawal symptoms from clomipramine can manifest as somatic distress, anxiety, and sleep disturbances.
  • Symptoms resolved spontaneously in one patient and with re-initiation of a low dose in another.
  • Differentiating TCA withdrawal from primary psychiatric disorder relapse is crucial.

Implications:

  • Clinicians should be aware of potential clomipramine withdrawal symptoms.
  • Patient education on gradual tapering may prevent adverse withdrawal reactions.
  • Further research is needed to fully characterize TCA withdrawal syndromes.
