Purpose of the Study:

  • To evaluate the short-term outcomes of interventional treatments for malignant obstructive jaundice.
  • To compare the efficacy of different interventional techniques.

Main Methods:

  • Retrospective analysis of 82 patients with malignant obstructive jaundice.
  • Interventions included percutaneous transhepatic biliary internal and/or external drainage (PTBIED) and percutaneous transhepatic insertion of biliary stent (PTIBS).
  • Etiologies included hepatocellular carcinoma, gallbladder carcinoma, hilar biliary carcinoma, pancreatic carcinoma, and metastatic carcinoma.

  • Significant reduction in total serum bilirubin (TSB) levels observed in most patients post-intervention.
  • TSB levels decreased from pre-operative averages of ~450 micromol/L to ~240 micromol/L at 1 week and ~90 micromol/L at 2 weeks post-PTBIED.
  • Similar bilirubin reduction was noted for PTIBS patients, with no significant difference between the two methods.
  • Short-term complications occurred in 33 patients, and 7 patients died within 30 days.

Conclusions:

  • Interventional therapy for malignant obstructive jaundice is a simple, safe, and effective treatment modality.
  • Both PTBIED and PTIBS demonstrate comparable short-term efficacy in relieving biliary obstruction.
