Summary

Laparoscopic-assisted transanal pull-through is effective for Hirschsprung's disease, especially after a colostomy for severe enterocolitis. This two-stage approach offers excellent outcomes, including early feeding and discharge.

Area of Science:

  • Pediatric Surgery
  • Gastroenterology
  • Surgical Innovation

Background:

  • Hirschsprung's disease requires surgical intervention to remove the aganglionic segment of the colon.
  • Laparoscopic-assisted single-stage transanal pull-through is a common primary treatment globally.
  • Late presentation and enterocolitis in India often preclude primary single-stage procedures.

Observation:

  • Three cases of Hirschsprung's disease treated with laparoscopic-assisted transanal pull-through following a prior colostomy are presented.
  • Two patients had severe enterocolitis, making a primary single-stage procedure infeasible.
  • The third patient underwent a diverting colostomy before referral for definitive treatment.

Findings:

  • The two-stage laparoscopic-assisted transanal pull-through procedure, performed after initial colostomy, yielded excellent results in these cases.

Related Experiment Videos

  • Histopathologic determination of the aganglionic bowel length guided the definitive procedure.
  • Successful outcomes included early postoperative feeding and hospital discharge.
  • Implications:

    • This technique provides a viable alternative for managing Hirschsprung's disease in resource-limited settings or when patients present late with complications.
    • The procedure offers good cosmetic results and facilitates faster recovery.
    • It highlights the adaptability of advanced surgical techniques to diverse clinical scenarios.
