Summary

Post-thrombotic syndrome, resulting from deep venous thrombosis, causes chronic leg symptoms. Lifelong conservative compressive bandaging is the primary treatment to manage symptoms and prevent ulcer recurrence.

Area of Science:

  • Vascular Medicine
  • Phlebology

Context:

  • Post-thrombotic syndrome (PTS) arises from deep venous thrombosis (DVT).
  • Characterized by chronic leg pain, edema, skin changes, and ulceration.
  • Caused by venous valve dysfunction and/or deep vein obstruction leading to venous hypertension.

Purpose:

  • To review the pathophysiology, diagnosis, and management of post-thrombotic syndrome.
  • To highlight the efficacy of conservative treatment modalities.

Summary:

  • Diagnosis involves assessing the muscular-venous pump function via plethysmography or dynamic phlebography.
  • Valvuloplasty has shown limited success.
  • Conservative compressive bandaging is the cornerstone of management, reducing symptoms, healing ulcers, and preventing recurrence.

Impact:

  • Emphasizes the critical need for lifelong adherence to compressive bandaging for effective PTS management.
  • Informs clinical practice regarding the primary treatment strategy for PTS.
  • Highlights the limitations of surgical interventions like valvuloplasty.
